Utilizing the Pre-Read read type for a radiology group enables some workflows which may be helpful in situations where the initial read is not intended to be sent to the healthcare provider. It is a private read type for radiology groups, and may be used as an internal state for the report before it is completed for the facility.
In order to enable pre-reads, a radiology group must first have it turned on for them and configured as per the read type documentation. Once that is done, a group administrator is able to set up the necessary permissions and configuration to make use of it. Note that after Pre-Reads are enabled, you will need to log in again in order to see the options.
This section will outline how the first physician will be able to create the Pre-Read report. Once complete, it will NOT be sent to the facility, but a report will exist in the system with a read type of Pre-Read. Facility users will NOT be able to see this report.
The facility we will use is North Haverbrook Imaging Center, and the physician who will do the Pre-Read is testrad1.
As shown in the pre-read read type documentation (see links above), the facility should be enabled for pre-reads as shown.
We will now edit testrad1's permissions and limit their ability to only perform pre-reads for North Haverbrook.
Now, when testrad1 goes to enter a report for North Haverbrook, it will only have the Pre-Read read type enabled and selected.
testrad1 will launch the report editor and enter the report as usual. In this screenshot they are shown to just be storing the dictation audio file, but the more common use case would be for them to be using either OnePACS or 3rd party voice recognition to create the Pre-Read Report.
Once the report is entered, testrad1 will click the "Complete" button. At this point, a pre-read report will be created and only viewable by the radiology group staff who have access to view reports for that facility.
If you would like the status to be set to "Dictated" after the report is completed, ensure that the radiologist has the "Change Study Status" permission. This will give them the ability to change the status of the study to "Dictated" when they complete. This is not required, but some groups may want this for a workflow step. Once that is in place, when they complete the report, they will see a selection option on the Complete button which will enable them to set the study status as "Dictated" |
Just as an example of what can be done here, we'll demonstrate a transcriptionist could come along and view all Pre-Read reports and save a Draft for final review/completion. This of course is not necessary if voice recognition was used. In that case, the finalizing radiologist could just view the report that the first physician entered.
A transcriptionist (or anyone who bases their workflow off of Pre-Reads) could set up a worklist tab with the following filter which will only show cases with a Pre-Read Report:
Again, note that Pre-Read reports are not visible to facilities. They will only see it as Ready to Read at this point.
This particular transcriptionist user does not have the ability to finalize or create reports. They only have the ability to create Drafts for North Haverbrook:
As a minimal permission set, the transcriptionist has the standard Clinician permissions, plus View Drafts. This could obviously be customized as needed.
The transcriptionist may log in, and click the Enter Report icon for this study with the Pre-Read report. They would then transcribe the audio, and click Save Draft:
As another example, in this case, we have given this user the permission to be able to assign studies, so after they create the draft they assign it to testrad2:
The point of a pre-read report is to have a complete report which is only accessible internally. The previous example of transcription serves only to provide a deeper workflow example.
The radiologist who will complete the report (in this case testrad2) could have their worklist filter set up for Pre-Read reports if that transcription workflow is not in place as shown above). In that case, they would simply go into the Enter Report screeen and complete the report, making any changes they feel are needed. In this situation we need the radiologist to have the correct read type. For example, this facility requires Prelim reads, so in order to complete reports they will need that to be configured for their facility privileges:
After completing the report, it will get distributed according to the report distribution rules.
To continue the transcription example which utilizes Pre-Read read types and Drafts, testrad2 will log in and have a tab set up which shows studies assigned to them with a Pre-Read read type. This will be their active worklist:
Clicking Enter Report will bring up the editor that they have configured for Prelim reads:
You'll notice that they it states there are 2 previous reports. The first is the Pre-Read (which in this case was saved as audio) and the second is the transcription which was saved as a Draft. The Draft text is shown, and the radiologist can make any needed changes. In this case, they will hit the lightning bolt to generate the report with the auto-selected template, and also receive auto-generated Impressions (which can be configured for the group).
After completing the report, it will get distributed according to the report distribution rules.
There are a few things to keep in mind with some Pre-Read workflows.
